Information: 

Frog record Label from Europe produced this great 
package in 1984, however it was severely limited to 20 numbered  copies on clear multi-colored vinyl with bonus 45 rpm.
The 45's were pressed on orange vinyl and in a black and white picture sleeve (version 1-1)
It was also released on black vinyl without the 45,
(version 1-2) in a quantity of 480 pieces.
In 1985 the piece was again released without the 45.
There were 40 copies pressed on white multi-colored vinyl. (version 2-1) There were also 160 copies on black vinyl released in a 'craft cardboard' type light blue-green  jacket (version 2-2) as is shown in photos above. The tracks and info are listed on the reverse, with all type upside down. The labels are blank white. 

This is the complete Dylan part of the show from Boston, Nov. 21 1975. It is a great sound. Mostly soundboard. Perhaps this readily available great sounding tape was the reason that so many vinyl versions of this show began appearing.
